The letter of intent to either the Activation Fund or the Health Care and Health Promotion Synergy Initiative should address the following:
·justification or rationale for the proposed project and explanation of its relevance to the goals of either the Foundation's Activation Fund or Synergy Initiative;
·brief explanation of the proposed project’s purpose or objectives, strategies or interventions, and project duration; and
·concise statement of the anticipated outcomes or results at the end of the project period (for Activation Fund grants, the results to be achieved within the 12 month period) or the anticipated results of the planning grant (for Synergy Initiative letters of intent).
The letter of intent should not exceed 3 pages, typed in no smaller than 12-point font, plus attachments.
The following six attachments to the letter of intent are required:
1. overall budget of the proposed project with amount requested;
·For Activation Fund grants this is the projected project budget
·For Synergy Initiative grants this should be the projected budget for the planning year, as well as an estimate of the cost of the entire project
2.relevant qualifications of those expecting to direct the proposed project and the capacity of the applicant organization to successfully manage the proposed project;
3.a copy of the applicant organization’s IRS letter indicating current tax exempt status;
4. most recent financial statement;
5. accountant's Report or Audit with Opinion Letter and Management Letter (a copy of the most recent Form 990-N (e-postcard) may be submitted if your agency is not audited);
6. a list of current officers and members of its board of directors.
Submissions, including all attachments, should be included in one email and sent to info@hfcm.org by 5:00pm on the date of the appropriate deadline.
All submissions will be acknowledged within 24 hours with an electronic receipt.If you do not receive an email, please contact the Foundation to confirm receipt of your submission.
