Oral Health Initiative of North Central Massachusetts (OHINCM)

Results

The OHNICM has been successful in achieving a number of its goals. These include:

• the establishment and expansion of dental services at three CHC, Inc.sites (in Fitchburg, Gardner and Leominster) and the ACTION Health Services program for the homeless and publicly housed populations. These four sites have served more than 20,700 patients in 20 dental chairs. Fifteen dentists, two hygenists and 17 dental assistants have provided this care.
• the provision of school-based dental services for grades K-5 at 16 schools in five area school districts. CHC, Inc. has received a grant from The Oral Health Foundation to expand this program to all grades (K-12) in these school districts.
• the establishment of the Dental Hygiene Program at Mt. Wachusett Community College which has graduated 21 students and is expanding enrollment in the fall of 2008. The students enrolled in this program have provided oral health services and education to more than 5,000 area residents.
• work with the Oral Health Advocacy Task Force has resulted in significant changes including the restoration of MassHealth adult dental health benefits, increased reimbursement rates for children and adults, funding for a third-party administrator for the MassHealth dental program; increased funding for DPH's Office of Oral Health and funding for the BEST Oral Health Program, a preschool oral health project. These changes have resulted in 47 North County dentists now accepting MassHealth patients when none were doing so at the inception of the OHINCM.

The Joint Coalition on Health of North Central Massachusetts received Planning (2001), Pilot (2002) and Implementation Grants (2003-2007) to expand access to treatment and preventive dental care in North Central MA.  Oral Health Initiative of North Central Massachusetts (OHINCM) has been successful in treating significant numbers of those who were without dental care – more than 16,700 individuals – or nearly half of those identified as in need of dental services at the start of this project.  For Year 5 of implementation, OHINCM will focus on sustaining their existing programming and recruiting additional dentists in private practice to participate in MassHealth.  They also will expand services for the elderly who do not have dental care, complete the transition of the School Based Oral Health Prevention Program to Community Health Connections (CHC), expand services to pre-schoolers by partnering with Project Apples in Leominster and add three more dental chairs at CHC's Fitchburg Dental Service.

The Joint Coalition on Health received Planning (2001), Pilot (2002) and Implementation Grants (2003-2006) to expand access to treatment and preventive dental care in North Central MA.  The Year 4 Implementation Grant reflects a continuation of successful strategies including:  CHC Dental Service in Fitchburg and Gardner; dental GHAP program in Fitchburg and Leominster; hygienist training program at Mt. Wachusett Community College; school-based sealant and fluoride varnish programs in five North Central school districts; training for school nurses, pediatricians, maternity nurses and child birth educators to conduct parent education programs; implementing a pre-school oral health education program; a public awareness campaign (“Watch Your Mouth”); and public policy advocacy in conjunction with Central Massachusetts Oral Health Initiative (CMOHI) and the statewide Oral Health Advocacy Task Force coordinated by Health Care For All.

New strategies proposed this year include adding three dental chairs at the Gardner satellite of the Community Health Connections Family Health Center; partnering with the Forsyth Institute to expand school-based services to include temporary fillings and increase the age range of children served; preparation for the 2007 dental residency rotation with UMass; and use of the GHAP model to recruit providers to serve MassHealth patients and enroll new patients as health care reform is implemented.

The Foundation funded the third year of Implementation of the Oral Health Initiative of North Central Massachusetts (OHINCM) through the Synergy Initiative, for $350,371.  The OHINCM will continue and expand its successful strategies, such as:  CHC Dental Service in Fitchburg (with plans for a 2007 dental residency rotation with UMass); dental GHAP program in Fitchburg and Leominster; fluoride varnish program in local elementary schools; training health care professionals and child birth educators to conduct parent education; public policy advocacy in coordination with the CMOHI and the statewide Oral Health Advocacy Task Force coordinated by Health Care For All; coordinating GHAP enrollment with HCAP outreach workers and Health Alliance Hospital with the aim of institutionalizing the program; implementing the 12-student dental hygiene program at Mt. Wachusett Community College and CHC; and expanding outreach education to summer youth programs.  The OHINCM recently celebrated significant contributions and pledges, in excess of $100,000, by dentists in North County to help establish the dental hygiene program at Mount Wachusett Community College.

The Oral Health Initiative of North Central Massachusetts (OHINCM) will continue its successful strategies, during the second year of implementation, to reduce the incidence of dental disease and increase access to preventive care and treatment in North Central Massachusetts.  The OHINCM will expand the capacity of the reduced-fee dental clinic at the Community Health Connections (CHC) Family Health Center by adding three chairs and implement plans for a satellite clinic in Gardner; increase outreach and marketing/promotion strategies to enroll residents from Leominster and Fitchburg in GHAP; expand the school-based fluoride varnish and sealant program to include Templeton Schools, along with schools in Fitchburg, Gardner, Winchendon, and Leominster; expand community education and outreach efforts to community-based organizations; implement the dental hygiene program and curriculum developed in coordination with Mt. Wachusett Community College and Community Health Center; and continue to coordinate advocacy efforts with Central Massachusetts Oral Health Initiative (CMOHI).

Partners include:  Wachusett District Dental Society, Heywood Hospital, CHC Family Health Center, Mt. Wachusett Community College, Health Alliance, and the Fitchburg, Leominster, Gardner, Winchendon and Templeton Public School Districts.

Joint Coalition on Health of North Central Massachusetts will implement a program to reduce the incidence of dental disease and increase access to preventive care and treatment in North Central Massachusetts.  With the support of earlier Planning and Pilot Grants from the Foundation, the Joint Coalition will expand upon its successful strategies to outreach to Leominster and Fitchburg residents to increase enrollment in GHAP; double the capacity of the reduced-fee dental clinic at the Community Health Connections (CHC) Family Health Center to 10 chairs, and expand clinical services to Saturdays; develop a curriculum and implement a dental hygiene program through Mt. Wachusett Community College; continue the sealant program for second graders in Fitchburg, Gardner and Winchendon and expand to include the Leominster Public Schools; expand community education and outreach efforts with local organizations; and coordinate with Central Massachusetts Oral Health Initiative (CMOHI) to recruit dentists to participate in MassHealth under the new caseload cap pilot.

The Montachusett Opportunity Council, along with the Joint Coalition on Health of North Central Massachusetts, will implement a comprehensive pilot developed with the earlier planning grant to:  add dental services at the Community Health Connections Family Health Center; coordinate with the Wachusett Dental Society and the Gateway Health Access Program to expand access to private dentists through a reduced fee program; provide sealants to second graders in the Fitchburg and Winchendon public schools; educate parents about children’s oral health; develop a local residency program for dentists through state dental schools and a training site for Mount Wachusett Community College’s dental hygienist program; coordinate with the Central Massachusetts Oral Health Initiative (CMOHI) to educate and advocate for legislative and regulatory oral health improvement initiatives.

The Joint Coalition on Health of North Central Massachusetts, the Gardner Area Interagency Team, and the Fitchburg/Gardner Area Community Health Network Area will work to define a model to increase access to dental care and provide dental health education and promote oral disease prevention in North Central Massachusetts.
